The old Giant Mushroom is an unobtainable food item. It was replaced by the Giant Mushroom in 0.4.7-4a21.
Prior to this version, Giant Mushrooms could be used to permanently increase the player's maximum health by 25 as many times as they were consumed; this meant that if enough were obtained, a player could build up an extremely unbalanced and unintended maximum health count.
This was patched in 0.4.7-4a21, and the old Giant Mushroom was replaced with the current Giant Mushroom, which instead will only increase the player's maximum health on the first use.
Any existing Giant Mushroom items that were already stored in a player's inventory or storage items would remain with the ability to increase maximum health several times, due to the fact this item and the current Giant Mushroom are separate items entirely. Beyond this, however, it is completely impossible to obtain this item in regular gameplay.
